Getting Started:

You are able to see a Rehab United physical therapist without a prescription from your doctor or specialist, but it is unlikely that your insurance company will pay for those services. To utilize your insurance, we recommend that you contact or see your medical provider and ask for a written prescription, i.e. primary care physician, orthopedist, and/or other specialist. You will need to visit your physician and be evaluated prior to you first physical therapy visit. Please note: If you prefer to privately pay for physical therapy services, simply contact our office for our cash-pay rates and packages and schedule an appointment.

Complete Paperwork:


Once you receive a prescription or a referral for physical therapy, call our office at 858-279-5570 to schedule an appointment. Our front office staff will ask some important questions regarding insurance verification, coverage, and personal information that will expedite the process. Have your insurance card ready. We will need the following:

    1. Your insurance card ID and group number
    2. Your birth date (and that of the subscriber if it is not yourself)
    3. Insurance customer service number on the front/back of your card

Schedule your first visit:

At Rehab United we will do our best to work with your schedule and accommodate your needs.Once you have scheduled your first appointment here are a few items of information that will help you.



  • Hours of operation are from 7:00 AM to 7:00 PM Monday - Friday

  • The evaluation will take approximately 1 Hour. Please arrive 15 minutes prior to your scheduled appointment to insure that all your paperwork is completed and correct.

  • Please dress in a way that will allow your therapist the best opportunity to evaluate you effectively. We recommend that your wear comfortable shorts, short sleeve shirt, and bring the shoes that you wear most often.
