Here's What To Expect:
For your first visit, we ask that you check in 20 minutes early to begin your appointment on time.
Step 1
Complete the required intake paperwork that gets sent to your email.
24 hours before your appointment, you will receive a functional outcomes survey from Limber. Please complete the survey before your appointment.
Step 2
In preparation for your appointment, wear comfortable clothing you can move in, as though you are going to the gym.
Step 3
Upon arrival, check in with the Front Office Team to review your submitted paperwork.
Step 4
You are on your road to recovery!

Documents
If you are unable to print and complete the documents, please arrive 30 minutes early to complete the forms in our office. Failure to bring the required documents will delay the start of your treatment.
Payments
If you have a financial responsibility, please be prepared to make payment upon arrival.
Confirmations
All evaluations must be confirmed. The Front Office Team will reach out two days before your evaluation to confirm it.
Cancellation
If you need to reschedule or cancel this appointment, please give us a 24-hour notice in order to avoid a late cancellation charge. You must do so by calling or texting the clinic; we do not accept cancellations through online forms.
Appointment requests are not finalized until scheduled directly by an office associate. While we do our best to respond as quickly as possible, please allow up to 24 hours for response times, or up to 48 hours if submitted on a weekend.
