Here's What To Expect:
For your first visit, we ask that you check in 20 minutes early to begin your appointment on time.
Step 1
Download & complete the applicable registration form above (using black text/ink only).
Step 2
Provide the following records upon arrival:

Photo ID

Insurance Card(s)

Referral from Doctor

Claims Adjuster Information (if applicable)

Medication List
Step 3
In preparation for your appointments:
Wear something comfortable, as if you are going to the gym.

Documents
If you are unable to print and complete the documents, please arrive 30 minutes early to complete the forms in our office. Failure to bring the required documents will delay the start of your treatment.
Payments
If you have a financial responsibility, please be prepared to make payment upon arrival.
Cancellation
If you need to reschedule or cancel this appointment, please give us 24-hour notice in order to avoid a late cancellation charge.
Appointment requests are not finalized until scheduled directly by an office associate. While we do our best to respond as quickly as possible, please allow up to 24 hours for response times, or up to 48 hours if submitted on a weekend.
